I, the undersigned parent or legal guardian, give permission for my child to participate in Voltage Lacrosse clinics, camps, and training sessions. I understand that lacrosse is a physical sport involving inherent risks, including but not limited to falls, collisions, contact with sticks or balls, and other injuries that may result in serious harm. I voluntarily assume all risks associated with my child’s participation and certify that my child is physically able to participate in athletic activity.

In consideration of participation, I hereby release and hold harmless Voltage Lacrosse, its owners, coaches, staff, and affiliated facilities from any and all liability, claims, or demands arising from injury, illness, or damages sustained during participation. I authorize emergency medical treatment if necessary and grant permission for photos or videos taken during events to be used for promotional purposes
